Our practices follow applicable privacy laws and referral agent privacy policy guidelines.

By using our services, you acknowledge and agree to the practices described in this Privacy Policy.

We explain why personal information is needed before requesting it. We collect personal

information only as necessary to:

● Understand care needs, preferences, and placement requirements

● Identify appropriate senior living and care options

● Facilitate referrals to selected senior living facilities

● Communicate with clients, families, and authorized representatives

● Comply with applicable legal and regulatory requirements


We do not collect personal information for unrelated purposes.


2. How We Collect Personal Information


We collect personal information by lawful and fair means, and only with your knowledge and

consent.

Personal information may be collected directly from:

● The client


● A legally authorized representative

● Family members or individuals involved in care planning, with the client’s consent


Information collected may include:

● Name, address, phone number, and contact details

● Care needs, medical conditions, and service preferences

● General financial resource information necessary to determine appropriate placement

options


Clients are responsible for ensuring their personal information is accurate, complete, and up to date.


3. Consent and Disclosure of Personal Information


Before sharing personal information with any prospective senior living facility, Oregon Senior Hope will:

● Provide a Disclosure and Advisory Form and Privacy Statement

● Obtain the client’s written affirmative consent


Personal information is shared only with facilities to which the client has agreed to be referred. We do not sell, rent, or share personal information for marketing or unrelated purposes.


4. How We Use Personal Information


Personal information is used only for the purpose of providing referral services, including:

● Evaluating care needs and preferences

● Identifying appropriate placement options

● Communicating with prospective facilities selected by the client

● Coordinating referrals and placement services


We do not use personal information for any purpose unrelated to referral services.


5. How We Protect Personal Information


We protect personal information using reasonable administrative, technical, and physical

safeguards appropriate to the sensitivity of the information. These safeguards help protect

against loss, theft, and unauthorized access, disclosure, copying, use, or modification.

Security measures include:

● Secure storage of physical records

● Secure electronic storage and password-protected systems

● Limiting access to authorized personnel only

● Confidential handling of all client information


While we use reasonable safeguards, no system can guarantee absolute security.


6. Retention and Destruction of Personal Information


Oregon Senior Hope retains personal information only as long as necessary to provide services and comply with regulatory requirements.


● Signed Disclosure and Advisory Forms are retained for three (3) years, as required.

● Health and financial resource information is not maintained long-term and is retained

only as necessary to provide referral services.

● When personal information is no longer needed, it is securely destroyed or permanently

deleted.


7. Client Rights and Requests


Clients have the right to request:

● Access to their personal information

● Corrections to inaccurate or incomplete information

● Updates to personal information

● Deletion of personal information, where permitted by law


We respond to requests in a timely manner and may verify identity before fulfilling requests. We will explain how requests related to personal information are handled.


8. Changes to This Privacy Policy


Oregon Senior Hope may update this Privacy Policy as needed to reflect changes in

regulations, services, or privacy practices. The most current version will be available upon

request.
